You must bring in this registration form and have it signed at Allyse's Bridal (in the University Mall in Orem, Utah) or at Allyse's Bridal (in The Meadows in American Fork, Utah) to qualify for the Wedding of the Year Giveaway.

While you are in Allyse's, try on wedding dresses and write down the dress you want on the application form. If you buy the dress and win the giveaway, the money you have paid for your dress and accessories will be refunded to you. The winning bride must wear an Allyse's Bridal wedding dress with no significant alterations made to the design of the dress. If you buy your dress at any other bridal salon in Utah and win the Giveaway, you will not be able to receive a refund of any kind for your dress and accessories.

The drawing for the Giveaway will be held at the bridal fair and premier showing of Made of Honor at the Larry H. Miller Megaplex 20 Theater on Saturday, May 3, 2008 at the District located at 11400 S. Bangerter Hwy, South Jordan.

Tickets for the event can be acquired by going to the Megaplex 20 Theatre in South Jordan or by going online to www.megaplextheatres.com. Pick the date May 3, 2008, then pick the event time which will be 4 p.m. The drawing will be held in a 580 seat theatre. Seats for the combined event and movie are limited. Any remaining tickets for the event can be sold day-of. If tickets are sold out, there will be standing room in a secondary location where a screen will be set up. Applicants can participate by watching the drawing and will be able to respond appropriately if they are the winner. Tickets are $7, which will include entrance to the bridal event with bridal vendors, refreshments, the Allyse's Bridal fashion show, the $50,000 wedding giveaway drawing, and the 5:15 p.m. showing of Made of Honor. Bridal fair tickets will be available to all showings of Made of Honor, but the $50,000 Wedding Giveaway will be held only during the posted 4 p.m. showing. Both the future bride and groom MUST be in attendance at the drawing in order to win. Applications received after May 2, 2008 will be eligible to win the 2009 Utah's wedding of the Year Giveaway Contest.

Also, in order to win, you must be engaged to be married with a wedding date after July 15, 2008. Winners must be flexible with their wedding date as numerous vendors with existing schedules must be accommodated.
